Access to Care

Available 24 hours a day, seven days a week, Medvivo’s Single Point of Access (known as Access to Care) is supported by our Urgent Care Coordinators. Our integrated care hub is fully manned by Access to Care clinical staff in office hours and daytime weekends, including Nurses, Physiotherapists, Occupational Therapists and Paramedics.

Altogether, we deliver an integrated rapid health and social care response service, Urgent Care @ Home (UC@H), including Domiciliary Care Provision, for service users in crisis in their own home. This service enables:

  • Clinicians to access appropriate assessments in the right place at the right time
  • Patients to remain at home with additional support
  • 'Step up’ care options to a community hospital or intermediate care bed
  • Patients to return home with additional support following acute assessment

In every case, our Access to Care Clinicians, Urgent Care Coordinators, Urgent Care Call Advisors, and Mobile Carers work together to provide a seamless service that puts patients first.
