Access to Care (ATC) is Medvivo’s Single Point of Access (SPA) service for Wiltshire only. The service has been commissioned by the Wiltshire Clinical Commissioning Group.
The service involves coordinating care delivery and managing patients through the healthcare system using a team that includes nurses, paramedics, occupational therapists and assistant practitioners.
As a remote clinical assessment service, the team helps to prevent acute hospital admissions and expedites discharges, while also providing support for referring the patient to community health teams.
Between the hours of 8am - 9pm (Monday to Friday) and 8am - 6pm (Saturday, Sunday and bank holidays) the integrated care hub is fully manned by dedicated ATC clinical staff. Outside these core hours the service is provided by Medvivo’s Out of Hours clinical staff.
