Advanced/Enhanced Clinical Practitioner (Care Coordination) - Part/Full time

We have an exciting opportunity for an independent, autonomous Prescribing Clinician to join Medvivo's award winning Care Coordination service on either a full or part-time, permanent basis.

About the service

The Medvivo Care Coordination service was established in December 2022, to test a new approach to coordinating care for patients, providing a means for system partners to work together with the ambulance trust to provide alternatives to hospital conveyance wherever appropriate.

Avoiding unnecessary hospital attendance and admission is a national priority with the Medvivo service being presented as a vanguard for future services. Our clinical leadership team provides support to areas across the UK in the establishment of their local Care Coordination services.  In addition, we are frequently approached for expert advice from agencies such as the NHS Confederation, and the Department of Health and Social Care Policy Unit on issues affecting the NHS frontline.

The success of the BSW Care Coordination service has been recognised nationally, demonstrated with the recent Health Services Journal Gold award for the most effective contribution to integrated health and care.  The service has now been extended to cover another Integrated Care Service within the South West.

Our service runs 7 days a week, Monday - Sunday (08:00-20:00)
